There are a number of steps that need to be taken in order to effectively disseminate a message out to the group you are trying to influence. Doris was an employee in a bank’s Demand Services Department.  She suffered from dysthymia, a form of depression, along with phobia and bouts of more intense depression. Over several years she was absent from work on a relatively frequent basis. The employer discharged her after continuing absences following two periods of probation for absences from work. Technology has changed drastically in the last decade and nurses have seen many changes around electronic medical records that directly impact their job performance. Thorough skin assessments play a major role in recognizing the initial stages of pressure injuries and the importance of implementing preventive measures to reduce breakdown. Our healthcare systems are directly impacted by hospital-acquired conditions such as pressure injuries and the importance of implementing prevention measures is top priority. Education is an ongoing challenge since you have nursing assistants, new graduates as well as nursing students that need to be informed of at-risk patients and what to look for regarding initial skin breakdown.

There have been several apps launched the last few years that has shown progress in helping staff recognize breakdown and useful technology for prevention and treatment of pressure injuries. Education should be ongoing for staff to help reduce HAPI’s, this type of technology is very useful to integrate as part of a “Pressure Injury Prevention Program”. The project facility that I am working with is looking to launch this app to help with facility acquired pressure injuries. Most staff carry and use phones throughout the day, this would be a useful tool that can be referenced when faced with decisions regarding prevention or treatment issues for pressure injuries. Nurses are very visual and seeing a photo and easy-to-use graphic was very useful in the launch stages. According to authors Salome and Ferreira, “the app was useful in clinical practice, helping to prevent pressure injuries and promote select nursing interventions for the treatment of patients with pressure injuries” (Salome & Ferreira, 2018).

Barriers to cultural sensitivity in the nursing profession can include stereotyping, discrimination, racism, and prejudice. There are situations in which we may portray a lack of sensitivity without realizing it or intending to offend someone else. Simple steps such as addressing patients by their last name or asking how they wish to be addressed demonstrate respect. Never make assumptions about other individuals or their beliefs. Always find out what the patient knows about their health problems and treatments. Understand where men and women fit in the patient’s society. In some cultures, the oldest male is the decision-maker for the rest of the family, even with regards to treatment decisions. Most importantly, make an effort to gain the patient’s trust for a stronger nurse-patient relationship (Anderson, 2012).

Culture competence is the ability to provide effective care for patients who come from different cultures. It requires sensitivity and effective communication in nursing, both verbally and non-verbally (Anderson, 2012).

Cultural Preservation : Nurses apply this concept by supporting the use of scientifically acceptable cultural practices in nursing. Nurses can understand these practices through interaction with the local communities in which they operate. This promotes the culture of the local community; the members of the communities feel a sense of belonging to their community and cultural identity. For example, Mwarobainini herb is believed to cure forty ailments in Kenya. 

Cultural accommodation: This is the support by nurses of cultural practices that are not harmful. For is believed in china that placing a metallic object on a fractured bone facilitates the recovery of the fractured bone. Although not proven harmful the practicality of this practice has not yet been proved however cultural accommodation encourages nurses to propagate such practices. This is a way of promoting the culture of the local communities.

Cultural repatterning: Some cultural practices are harmful to the patient. For example, some herbs can cause harm to patients when used, for example the Msemei herb which has been used by medicine men in Ghana to cure cough in children has been proved to be harmful to health. Cultural repatterning involves the nursing discouraging the use of cultural practices that have been proven harmful. 

Cultural brokering: This is the use of cultural practices combined with the health care practices to improve the healing process of the patient. Some herbs used in some communities have been proven to cure some diseases. The nurses may use such herbs in conjunction with the health care plan to improve the healing process.

Practice of the above strategies can be hindered if nurses lack knowledge of the cultural practices of the communities they serve. Nurses should therefore posses’ knowledge of the cultural practices of their communities because some of them may be helpful in healthcare (Paulanka, & Purnell, 2005).

Nurses in general health care settings have found culturally diverse patients to be challenging and frustrating. Specific difficulties identified have been language barriers, dealing with relatives, barriers within the bureaucratic system, and personal stress and helplessness within the nurse-patient relationship (Medscape Log In., n.d.).

Imagine that it is late at night and you wake up suddenly, struggling to breathe. You realize immediately that your symptoms are severe—this is not a common cold. Unsure of what to do, you call a loved one who insists on driving you to a hospital emergency room. Assume, for the purposes of this example, that you are a U.S. citizen. While you are registering for emergency services, you notice a sign prominently displayed, declaring that you have the right to medical screening and stabilization in the case of emergency or labor, even if you do not have health insurance. This is a provision of the Emergency Medical Treatment and Active Labor Act (EMTALA), which you explore in more detail in this week’s reading.

You may be surprised to learn that Americans have no legal right to health care, as established through extensive legal precedent. In fact, EMTALA represents the only health care rights guaranteed to Americans.

The United States is one of the only high-income nations that does not guarantee health care as a fundamental right, and it is the only developed nation that has not implemented a system for insuring at least all but the wealthiest segment of its population against healthcare costs. . . . In terms of national constitutions, a 2004 survey reported that some two-thirds of constitutions address health or health care, and that almost all of these do so in universal terms. (Teitelbaum and Wilensky, 2013, p. 108)

However, according to the U.S. Census Bureau (2013), 15% of the U.S. population was uninsured in the years immediately preceding the Patient Protection and Affordable Care Act (PPACA). Therefore, except in specific situations (such as those regulated by EMTALA), 48 million people did not have access to health care. Furthermore, according to the same report, family income was strongly correlated to a family’s access to health care. Whereas 75.1% of families with an annual income less than $25,000 were insured, 92.1% of families earning $75,000 or more per year were insured.

These figures may trouble you from a social perspective, but remember that there are many stakeholders in public health. A patient’s perspective is but one of the many you must consider as a public health professional. In this Discussion, you explore whether health care is a right or a privilege.

Question Description

Baptist Hospital is a 175-bed community hospital serving a rural area of Jacksonville.  It is located 30 miles from a large metropolitan area and has a fine reputation.

 The hospital is fully accredited and is licensed by the state.  The hospital offers general medical/surgical services with a bed complement as follows:

The hospital has a 10 bed Ambulatory Surgical Unit.  The hospital is in sound financial condition with a physical plant that is 15 years old and in excellent condition.

The hospital has just recruited a new General Surgeon and a Internist.  These recruitment efforts filled two major needs of the institution. It is estimated that in today’s current marketplace surgeons are expected to increase inpatient admissions to a acute care community hospital by 1.5% and Internists by .5%.

 It is expected that admission volumes will increase with the addition of these two new physicians.

The primary initiative or goal for the 2014-2015 fiscal year is the establishment of a 15-bed Psychiatric Unit. This unit is expected to occupy space for 15 beds currently not set up and staffed at the hospital.

It is expected that during the year there will be a slight increase in inpatient services as a result of the Psychiatric Unit.  The hospital laboratory is buying two new pieces of equipment at a cost of $400,000 that will allow them to do 35% more tests in house rather than send these tests out to a reference lab. It is also expected that the Radiology Department is purchasing a new CT scanner that is expected to increase current radiology volumes by 3.25%.
